What kind of personal information does Canberra Men's Centre collect and how does it collect it?

support in their homes or elsewhere in the community or other forms of assistance: job applicants, staff members, volunteers and contractors.
Personal information you provide
Canberra Men's Centre will generally collect personal information held about an individual by way of face to face meetings, interviews, telephone calls, and forms filled out by the individual.
Personal information provided by other people
In some circumstances Canberra Men's Centre may be provided with personal information about an individual from a third party, e.g. a reference about an applicant for a position, a worker from another agency making referrals to Canberra Men's Centre services.
Exception in relation to employee records
This policy does not apply in relation to the treatment of an employee record, where the information is directly related to a current or former employment relationship between Canberra Men's Centre and the employee as these records are exempt from the application of the Commonwealth Privacy Act.
How will Canberra Men's Centre use the personal information you provide?
Canberra Men's Centre will use personal information it collects from you in order to provide a service (the primary purpose of collection). It may also, from time to time, use information for other reasons (secondary purposes) that are related to the primary purpose of collection; to which you have consented; or for other reasons as specified in the Privacy Amendment (Private Sector) Act 2000.
Clients
Canberra Men's Centre's primary purpose of collection is to enable it to provide services to men, and, on occasion, women and family members. Personal information can be used for:

Providing support to men in personal, relationship and family matters

Helping men develop and enhance their skills in relationships with partners or in caring for their children;

Identifying areas of risk for clients and families, as well as Canberra Men's Centre staff and volunteers;

Day to day administration;

Satisfying Canberra Men's Centre's legal obligations and allow us to discharge our duty of care.
In some cases where Canberra Men's Centre requests personal information and the information is not obtained, Canberra Men's Centre may be limited in the extent to which it can provide a required service. Where Canberra Men's Centre is collecting information, it will issue a copy of the 'Standard Collection Notice'.
Job applicants, staff members, volunteers and contractors
The primary purpose of collection is to assess the suitability of applicants and employ or engage an applicant judged as able to appropriately fulfil Canberra Men's Centre's requirements for the role. The purposes for which Canberra Men's Centre uses this information include administering the person's employment or contract; insurance purposes; seeking funds, and satisfying Canberra Men's Centre's legal obligations, e.g., in relation to child protection legislation. All applicants for jobs will be sent a copy of the 'Employment Collection Notice'. All new contractors and volunteers will be sent a modified version of the 'Employment Collection Notice'.
Who might Canberra Men's Centre disclose personal information to?
Subject to the Privacy Act, Canberra Men's Centre may disclose personal information held about an individual to another service provider; a Government department; a medical practitioner; people providing services to Canberra Men's Centre; your spouse/partner; or anyone you authorise Canberra Men's Centre to disclose information to.
Canberra Men's Centre will not send personal information about an individual outside Australia without obtaining the consent of the individual (in some cases this consent will be implied), or otherwise complying with the National Privacy Principles.
How does Canberra Men's Centre treat sensitive information?
In Canberra Men's Centre's context, sensitive information could mean information relating to a person's parentage, racial or ethnic origin, political opinions, religion, trade union or other professional or trade association membership, sexual preferences or criminal record. Sensitive information will be used and disclosed only for the purposes for which it was provided or a directly related secondary purpose unless you agree otherwise, or the use or disclosure of the sensitive information is allowed by law.
Management and security of personal information
Canberra Men's Centre's staff are required to respect the confidentiality of the information and privacy of individuals. We have in place steps to protect the personal information Canberra Men's Centre holds from misuse, loss, unauthorised access, modification or disclosure by use of various methods including locked storage of paper records and passworded access rights to computerised records.
Updating personal information
Canberra Men's Centre endeavours to ensure that the personal information it holds is accurate, complete, and up to date. A person may seek to update their personal information held by Canberra Men's Centre by contacting the Manager of Canberra Men's Centre at any time. The National Privacy Principles require Canberra Men's Centre not to store personal information longer than necessary for our purposes for use and disclosure.
You have the right to check what personal information Canberra Men's Centre holds about you
Under the Commonwealth Privacy Act and with some exceptions, an individual has the right to obtain access to any personal information, which Canberra Men's Centre holds, about them and to advise Canberra Men's Centre of any perceived inaccuracy.
To make a request to access any information Canberra Men's Centre holds about you, please contact the Canberra Men's Centre Manager in writing. Canberra Men's Centre may require you to verify your identity and specify what information you require. Canberra Men's Centre may charge a fee to cover the cost of verifying your application and locating, retrieving, reviewing and copying any material requested.
